Start Here: Is Your Medical School Eligible?
Before anything else, your medical school must meet one specific requirement. NMC recognition alone is not sufficient. The eligibility check takes two minutes and must happen before you plan anything else.
If your school is not listed in WDOMS with an ECFMG Sponsor Note, you are not eligible for the US residency pathway. Verify before proceeding.

The Six Steps to a US Residency

Verify Eligibility
Confirm your medical school is listed in WDOMS with an ECFMG Sponsor Note. This is a non-negotiable first step.

Pass USMLE Step 1
Pass/Fail since January 2022. Tests foundational basic sciences. Available at Prometric centres across India. Can be attempted before MBBS completion.

Pass USMLE Step 2 CK and Obtain ECFMG Certification
Step 2 CK is the most critical scored exam for IMG applicants. Target 245 or above. ECFMG Certification also requires completing an ECFMG Pathway (usually Pathway 6 for Indian doctors) and passing OET Medicine.

Build US Clinical Experience
Observerships or externships at US hospitals. Secure Letters of Recommendation from US-based physicians who have supervised your work. Typically three LORs required.

Apply through ERAS and Register on NRMP
ERAS applications open on September 3 each year. Register separately on NRMP and submit your Rank Order List (deadline: March). Interview season runs October through January.

Match Day
Results announced in March. If unmatched, the SOAP (Supplemental Offer and Acceptance Program) offers a second chance at unfilled positions.
The USMLE Examinations: What Indian Students Need to Know

ECFMG Certification: Your Licence to Apply
ECFMG Certification is mandatory before you can participate in the Match. Think of it as the international equivalency stamp that US residency programs require from every non-US graduate.
- Pass USMLE Step 1
- Pass USMLE Step 2 CK
- Complete an ECFMG Pathway. This confirms your medical education meets ECFMG standards. Pathway 6 is the most common for Indian graduates.
- Pass OET Medicine (Occupational English Test), which assesses English proficiency in a clinical context.
Pathway note: Pathway 6 is the standard route for Indian medical graduates. Your counsellor will confirm which pathway applies to your specific school and graduation year.
Visa Pathways for Residency Training

Common Questions
Can I take USMLE exams in India?
Yes. USMLE Step 1 and Step 2 CK are both available at Prometric test centres in Ahmedabad, Bangalore, Chennai, Hyderabad, Kolkata, Mumbai, and New Delhi.
Do I need to complete my MBBS before starting?
No. You can register for USMLE Step 1 after completing your basic science phase. You do not need to wait for MBBS completion.
Is FMGE required for US residency?
No. FMGE is the licensing exam for foreign graduates who wish to practise medicine in India. It has no role in the US residency process.
What if my medical school is not in WDOMS?
You are not eligible for the US residency pathway. WDOMS listing with an ECFMG Sponsor Note is a non-negotiable requirement. Verify at wdoms.org before making any plans.
How long does the full pathway take?
Most Indian MBBS graduates take 2.5 to 5 years from beginning USMLE preparation to starting their residency, depending on exam performance, chosen specialty, and Match cycle.
Is age a barrier for IMG applicants?
Age is not a formal eligibility barrier. However, some programs are sensitive to large gaps between graduation and application year. Early planning reduces this risk.
What is the difference between J-1 and H-1B for residency?
J-1 carries a 2-year home-country return requirement but is easier to obtain. H-1B has no return obligation but is employer-sponsored and subject to an annual cap. For Indian doctors focused on long-term US settlement, H-1B is often the preferred route. Discuss with your counsellor.
